Heading and anchor rules
V1 recognizes ATX and Setext headings outside fenced code blocks. It removes common inline Markdown when forming labels, generates lowercase GitHub-style anchors, and adds -1, -2, and later suffixes to duplicate slugs. Indentation follows the heading level relative to the shallowest selected heading.
Input is limited to 200,000 characters. Anchor behavior is documented and may differ from a renderer with its own slug algorithm.
