First page of the actual Letter PDF with labeled writing areas for goal, why it matters, target date, milestones, actions, obstacles, and support.
Goal-plan page rendered from the downloadable Letter PDF. The second page is a progress review.

How to use it

Describe one goal in concrete terms, then name the evidence that would show it is complete. Break the work into milestones and the first actions you can take. Revisit the review page whenever you choose and adjust your plan rather than treating the original dates as fixed.

Privacy and limits

The worksheet is a blank download; this page collects no goal or personal details. It is not a clinical, therapeutic, or guaranteed behavior-change program. SMART-style thinking can fit inside these fields, but no separate SMART-goals page is needed.
